I think this one is newer, but it has already happened twice, and I had to re-load the game both times.<br/>

If you are at a spot where an encounter is supposed to happen, and you are waiting on dialogue to hit ok, a wandering monster can come in and then the fixed encounter will not show up.<br/>

Examples:<br/>
In Loch Caer, when you go to help lizardmen vs. orcs, I was in process of hitting Yes assist, when a wandering encounter happened.  Then after, the yes assist was still there.  I hit yes, no monsters.  I go away, come back, hit yes, no monsters.<br/>

In grave mound, axeman comes to life, before i can hit ok, wandering monster shows up, and same as above.  No way to make the guy come back.
